Team Roster Form  Use this form for each regional event you attend.  "This form contains a place for T-Shirt Sizes."

Nationals Team Roster Form Use this form for the Texas Open Nationals.  "This form contains a place for Champion Jacket Sizes."

Code of Conduct Form A Code of Conduct Form is required on file by Each Coach representing a performing team.

*Team Release/Waiver Form NEW! Form! "Less paperwork. Coaches can elect to use this form instead of the Individual Waiver Form."

*Remember use either the Team or Individual Release/Waiver Form for team members. DO NOT Send in Both Form

*Individual Release/Waiver Form "This form can be used for EACH individual instead of using the Team Form."

Credit Card Authorization Form "Coaches complete this form if you are paying using a CC". This Form MUST accompany Registration Form when Faxing.
